Please read this carefully before checking the box. By accepting, you are giving up certain legal rights.

The activity. The MVP Custom Jump Blueprint™ is an at-home plyometric and strength training program involving jumping, bounding, and bodyweight exercises. Physical activity of this nature carries an inherent risk of injury, including but not limited to sprains, strains, fractures, joint injuries, and in rare cases, more serious harm.

Medical clearance. By accepting these terms, you confirm that the athlete has no known medical condition, injury, or physical limitation that makes participation in vigorous physical activity unsafe, and that you have consulted or will consult a qualified healthcare provider if any doubt exists. If the athlete experiences pain, dizziness, difficulty breathing, or any unusual symptom during training, they should stop immediately and seek medical attention.

Assumption of risk. You voluntarily and knowingly accept all risks of injury, illness, or loss associated with the athlete's participation in the Program, whether caused by the negligence of Monster Vertical Power or otherwise.

Release of liability. To the fullest extent permitted by law, you release, discharge, and hold harmless Monster Vertical Power and its owner(s), employees, agents, and representatives from any and all claims, demands, damages, costs, or liability arising from the athlete's participation in the Program.

Parent/guardian representation. If the athlete is a minor, you represent that you are their parent or legal guardian, that you have the legal authority to execute this waiver on their behalf, and that you do so voluntarily.

Acknowledgment. You confirm that you have read this document, that you understand it, and that you accept it freely without any pressure or inducement to do so.

Governing law. This waiver is governed by the laws of the State of North Carolina.
